Beginning in 2002, the automated database generates the medical release roster. The information is directly based on parent input. To insure a child may receive medical assistance in the absence of the parent or guardian, on the first day of practice:
1. Ask the parent or guardian for each player to review the child’s medical data on the Medical Roster Form.
2. Ask each parent or guardian to sign the copy of the Medical Release Roster Form.
3. Keep a copy of the Medical Release Roster Form with the coach and the assistant coach. Make additional copies as needed.
Remember, if you lose or gain a child pre-season or during the season, medical release information for the new child must be obtained from the parent.
